To fill in this form you need to have chosen a pre-arranged Field Test day that you wish to attend. You should also have decided on the category, and grade, of test you wish to enter for, and have registered your dog* You will also need to have read the terms and conditions of booking. The form must be filled in by the Handler,or the Handler's parent/guardian if the Handler is under 18 years of age.** PLEASE COMPLETE ALL FIELDS

*you do not need to wait for your registration documents to arrive. You can enter a test as soon as you have sent us your registration form and fee. You can even pay your test entry fee and registration fee at the same time. **the Handler is the person who will be working the dog in the test.
