To fill in this form you need to have decided on the category, and grade, of test you wish to enter for, and have registered your dog* You will also need to have read the terms and conditions of booking. The form must be filled in by the Handler,or the Handler's parent/guardian if the Handler is under 18 years of age.**

*you do not need to wait for your registration documents to arrive. You can enter a test as soon as you have sent us your registration form and fee. You can even pay your test entry fee and registration fee at the same time. **the Handler is the person who will be working the dog in the test.

***This is the name of the person that is named as the owner on the Gundog Club registration documents. This may or may not be the same as the Handler.
